The August 2013 issue of the Drug Safety Update carried a warning on the use of oral ketoconazole.

Due to the high risk of hepatic toxicity (ranging from hepatitis to cirrhosis/liver failure) it advised against the use of oral ketoconazole completely.

This does not affect the prescription of topical ketoconazole, which is still considered safe to use due to it's low systemic absorption.
